Dementia Resource Guide

The Brown County Dementia Friendly Community Coalition, with the financial support of the Forget Me Not Fund, has created a Dementia Resource Guide for people with dementia, caregivers, professionals, and the community to use when a diagnosis of dementia is given. The Dementia Friendly Community Coalition works to raise awareness, develop cooperative partnerships, educate, and engage all to create a dementia friendly community that enhances the quality of life. We believe that by transforming the community, we promote inclusion, reduce stigma, and instill hope.

Inside the guide, you will learn where you can find resources regarding advance directives, importance of support systems, respite and in-home support, medication and treatment options, education about dementia, safety in the home, avoiding and responding to a crisis, funding and financial assistance, and end of life. This Dementia Resource Guide will not list all possible resources available, nor is it intended to replace a conversation with ADRC about your options, but it is a great place to start.
