The transition from school to the adult world is a significant milestone for any young adult, and quite frankly, it can be an intimidating period. This transition takes on special importance for youth with disabilities. When high school ends, so does their entitlement to special education and related services. Young adults with disabilities and their families must pursue and coordinate needed services and support for themselves.
ADRC is here to help you navigate topics including:
• Discussion of rights and responsibilities when turning eighteen
• Decision-making supports and guardianship
• Continuing education and/or employment
• Access to adult long-term care programs
• Information on applying and appeal for public benefits, including healthcare and disability
• Healthcare changes and insurance options

Our mission at ADRC is to “empower and enrich the lives of older adults, adults with disabilities, and their caregivers.” The earlier one becomes familiar with the tools that will help with this journey, the better off families and the individual will be. An ADRC Specialist can provide options counseling and address the needs of young adults as early as 17 years and 6 months of age. During an options counseling session, the specialist will get to know you, learn from you what your goals and aspirations are, address any questions or concerns, and assist in exploring services and supports that will best cater to your goals.
ADRC has a working relationship with the school systems as well as Brown County Children’s Services and can help provide a bridge to the adult world. ADRC can provide phone consults, in-person office visits, or home visits.
