# Home Delivered Meals

More than just a meal!

#### Meals are delivered by friendly volunteers to homebound older adults throughout Brown County.

### **Credit Card Payment Update:**

Beginning Thursday, August 1, 2024, all credit card transactions with ADRC & Grounded Cafe will be charged a processing fee. This processing fee helps us cover the cost associated with payments made with credit cards.

Enjoy a hot, nutritious lunch from the comfort of home!

Persons receiving a meal say it helps them continue living in their home.

## It’s more than just a meal.

Along with lunch comes a short visit, wellness check, and links to other services and support.

## Am I eligible for a meal\*?

You may be eligible for meals if you are:

- Age 60 or older, for the most part homebound, and unable to safely prepare meals on your own
- The spouse of an eligible person who is receiving meals
- Adult with a disability living with an eligible person who is receiving meals

*\*Adults under age 60 may qualify. Meals provided to FamilyCare or IRIS applicants with a meal authorization. Please call.*

## How do I sign up?

Call ADRC (920) 448-4312 to schedule a visit to meet with you in your home.

##### What's for lunch?

##### I’m on Family Care/IRIS, can I get meals delivered?

Meals are available with a service authorization.

Work with your Care Manager/Care Consultant to include meals in your care plan.

##### How long can I get meals delivered?

Meals are available on a long-term or temporary basis depending on your needs. Some receive meals short-term while recovering from illness or surgery, while a caregiver is out of town, or when driving is more difficult, like in winter.

Or you can receive meals long-term, helping you stay in your home so you or your caregiver have one less meal to shop for and prepare.

##### When and where are meals delivered?

Meal delivery is around the noon hour and is available Monday – Friday in most parts of Brown County, including:

- Green Bay
- De Pere
- Pulaski
- Denmark
- Howard
- Parts of Ashwaubenon
- Suamico (Tuesday and Thursdays only)
- Wrightstown (Tuesday and Thursdays only)

##### Can you provide for my special diet?

We serve a general diet. Scroll down to see what’s for lunch.

Special diet meals (low-sodium, texture modified, and gluten free meals) are not available. To learn more about services for special dietary needs, call us at (920) 448-4300.

All meals contain grains, protein, 1% milk, at least 3 servings of fruits and/or vegetables, and dessert most days. We try to limit sodium content by not adding salt during cooking, but some processed items will be higher in sodium.

##### Is there a cost?

Meals are available on a contribution basis for eligible customers. The suggested contribution is $5.00 per meal. Contributions are voluntary and confidential. No eligible participant will be denied meals based on their ability to contribute.

*FamilyCare or IRIS participants can get meals provided if they are included in your care plan and invoiced to the Managed Care Organization.*

##### How do I sign up for homebound meals?

You or your caregiver can call us at (920) 448-4300 or complete the [HDM Interest form](https://forms.office.com/pages/responsepage.aspx?id=37vT1LEj50CG0u9c0r24FzPIJDsF4-ZIm8PQb5x4-cBUQktFNDdaNkZFN0xFNUowNzRaQVNKSjM4Ti4u) and we will schedule a visit to meet you in your home.

All applications are subject to eligibility criteria review and route availability.

Approved applicants can begin meals in as little as two days.
