## Finding and Connecting You to Services

Persons with disabilities and their caregivers are often frustrated by obstacles in their daily life that others will never face.

So many challenges, questions, forms, agencies, doors. Where to start?

We want to learn about your hopes, dreams, and preferences.

Whether you are struggling, or caring for someone who is, we work with you to come up with a plan.

We search through available resources, put together options so you can make informed decisions, and then help connect you to the services you choose.

This may include a benefits check-up, to uncover what you are eligible for.

**Have you been denied benefits and need to appeal a decision?**  
We can help you understand your rights, discuss your options, and navigate the appeal process.

Together we form a team working with you to put your plans in motion to help you live your best possible life.
